Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Financial Information

We at OHM dental are dedicated to improving the smile and health of everyone. In order to help accomplish this, we offer a wide range of financing options that will meet nearly any budget. We accept a variety of PPO plans and many of our treatments are covered by insurance. Please reach out to your insurance carrier to find out more about your specific insurance plan and what is covered. For your convenience, we also accept Visa, Mastercard, Discover, and American Express. We also offer multiple financing options like cherry, sunbit, and proceed. 

Cancellations 

We realize unexpected circumstances arise and you cannot keep your appointment. If this occurs, we ask that you give us 48 hours notice to cancel or reschedule your appointment. We will charge $75 for a no show or a last minute cancellation
